Background of the assignment :

COVID-19 has exposed a significant gap in national capacities to provide oxygen when needed across the world. For most developing countries, this is mainly because of limited oxygen infrastructure and production capacity. Yet, health facilities need regular oxygen supplies for critical patients. At the same time, most countries lack trained human resources, including biomedical engineering staff to install and maintain oxygen equipment and qualified clinicians to administer safe oxygen therapy.

Although the significance of medical oxygen therapy has become much better known during the COVID-19 pandemic, the need extends across the health system especially for Neonatal and Intensive Care as well as for surgeries and other medical conditions. The situation in Botswana is no different and the country experienced a significant shortage to medical oxygen during this pandemic. Further, apparent gaps on oxygen quantification and use were noted in country.

 

Purpose of Activity/Assignment :

UNICEF Botswana is seeking the services of a consultant to provide technical assistance to the Ministry of Health to develop and implement a national oxygen roadmap, with specific focus on the following components :

  1. Conducting oxygen ecosystem analysis and mapping across the country ;
  2. Developing a clear implementation plan which details the requirements to supply medical oxygen in key district and referral hospitals ;
  3. Develop a framework for maintaining quality assurance, regulatory compliance, and alignment with standard medical and industrial practices ;
  4. Capacity building of core health staff on to programmatic use of oxygen ;
  5. Strengthening capacity of biomedical engineers to support oxygen infrastructure development, oxygen sourcing and maintenance planning.

 

Scope of Work :

The scope of the roadmap development includes :

  1. Develop an assessment protocol that outlines assessment methodology, key data sources/informants and resource need (leverage existing tools such as the UNICEF OSPT) ;
  2. Acquire and/or develop necessary data collection tool formats (leverage existing tools from UNICEF Global Oxygen Team including assessment tools for facilities and oxygen sources) ;
  3. Coordinate data collection ;
  4. Share data with UNICEF country and global teams for ongoing quantification work ;
  5. Baseline assessment including oxygen quantification (e.g., based on detailed analysis using UNICEF Oxygen System Planning Tool or similar). Map partners (GOs; UN, INGOs, private sector) that are relevant to the oxygen arena ;
  6. Analysis on human resources availability/capacity as well as other components of the oxygen ecosystem ;
  7. Comprehensively map relevant programmes and partners that are relevant to oxygen systems implementation ;
  8. Plan for procuring the oxygen sources (e.g., contracting with liquid oxygen manufacturers, procurement of PSAs) ;
  9. In close consultation with MoH, UNICEF Country Office and other key stakeholders/partners, develop the draft of the costed national oxygen roadmap ;
  10. Support a series of reviews of the national oxygen roadmap based on inputs from various stakeholders to produce a final version.

The scope of the implementation support includes the following activities utilizing the oxygen system planning tool :

  1. Identify target geographical areas and facilities (using UNICEF OSPT and/or related tools) ;
  2. Quantify the oxygen delivery systems and oxygen equipment needs and gaps in the targeted geographical areas and facilities ;
  3. Determine infrastructure modifications that would be required for utilization of the oxygen sources (e.g., vacuum insulated evaporators (VIEs), piping, building modifications for PSAs or LMO installation, electrical connections….) ;
  4. Plan for distribution of oxygen throughout the catchment area (e.g., procurement requests, oxygen provisioning–cylinder filling or liquid oxygen delivery or PSA piped oxygen, transport, and payment plans) ;
  5. Plan to manage/forecast the supplies needed for delivery of oxygen to patients.

The scope of biomedical engineering technical support includes :

  1. Provide technical oversight and quality assurance of the installation, testing & commissioning of medical oxygen (either PSA or liquid oxygen or cylinder or concentrator-based systems or a combination approach) ;
  2. Setting up or reinforcing existing quality assurance mechanism for continued quality assurance through regular on-site supervision and quality assurance visits during the project ;
  3. Adapt existing training modules and deliver training with aim to build local capacity and create master trainers on updated oxygen treatment guidelines and monitoring/forecasting of oxygen needs ;
  4. Develop a costing plan for oxygen sourcing for all 18 District Health Management Teams

The consultant should be based in Gaborone and will be expected to be on-site at Central Medical Stores and UNICEF during the data collection period. Visits to other stakeholders in targeted geographical facilities during the data collection period may also be required. The consultant will be required to support field work outside Gaborone as applicable.

Desk review report produced and to include:
Baseline assessment, Quantified oxygen demand and gap in supply at Central Medical Stores (CMS) and District Health Management Team (DHMTs) and referral hospitals. Analysis on HR situation, number of existing biomedical technicians, numbers of trained staff on medical oxygen use.

II. Provide training and mentorship to increase capacity around medical oxygen

Facilitate a series of national and district master trainers, training of trainers, and mentors from relevant health facilities and procurement agencies on how to manage oxygen (from procurement to operations and maintenance); and how to use the oxygen system planning tool, considering training on the UNICEF OSPT and UNICEF Oxygen ecosystem rapid assessment tool, as appropriate.

Training and mentorship plan materials adopted and adapted;
20 master trainers trained in oxygen management and use as well as maintenance of oxygen equipment in the north and the south of the country.

3 training sessions to train 80 ToTs in DHMTs on oxygen management facilitated;

Administrative issues :

The selection and conditions of service of consultant will be governed by and subject to UNICEF’s Policies and General Terms and Conditions for individual consultants/contractors.

No contract may commence unless the contract is signed by both UNICEF and the consultant/contractor.

Consultant/Contractor will be required to complete mandatory online courses (e.g. Ethics, Prevention of Sexual Exploitation and Abuse and Security) upon receipt of offer and before the signature of contract.

Consultant/Contractor will be required to sign the Health Statement for consultants/contractors and to share an applicable proof of health insurance covering medical evacuation (if travel is involved) prior to taking up the assignment.

To qualify as an advocate for every child you will have…

Education : Bachelor degree in Biomedical Engineering or relevant health care field with experience in health systems strengthening.

Experience :

  • Minimum of 4 years of relevant work experience ;
  • Previous experience in working and supporting at policy level around assessment planning, medical equipment optimization, strategy/roadmap development ;
  • Exposure and experience how medical equipment supply system and maintenance works ;
  • Computer literacy and willingness to be trained on UNICEF-specific oxygen tools including OSPT required ;
  • Excellent communication skills (both written and verbal) to be able to liaise with a wide variety of people; good attention to detail ;
  • Familiarity and experience of working with UNICEF and UMICs is considered an asset ;
  • Fluency in written and spoken English is essential ;
  • Local language proficiency is considered an asset ;
  • professional experience in qualitative and quantitative research, ideally in health or another social sector ;
  • Proficiency in data collection, processing and analytics, including Microsoft Word and Excel ;
  • Ability to deliver high-quality results and function effectively independently and within a partially remote team ;
  • Excellent oral and communication skills ;
  • Fluency in English and Setswana.
