Description de la mission

1) Overall assigment objective

Support to the Ministry of Health in the development of the project proposal on Human Resources for Health for L’Accélérateur in relation to the “4×4” reform

2) Specific Objectives:

Specific Objective 1: Support the need assessment and priority setting exercise in relation to the envelope available, current state of the HRH situation in the country and strategic priorities of L’Initiative/L’Accélérateur

Specific Objective 2: Support the grant writing (narrative documents, logical framework, budget and annexes) including a gender component

3) Expected outcomes

In relation to Specific Objective 1:

Expected Outcome 1.1: The literature review is completed – official documentation from the MoH, international reports and guidelines on HRH (incl. gender, integration of the three pandemics HIV/Malaria/Tuberculosis and Sexual and Reproductive Health and Rights issues), existing/previous feasibility studies, L’Initiative strategic documents and guidance notes, scientific and grey literature available.
Expected Outcome 1.2: Clear priorities are defined to establish the blueprint of the future project

In relation to Specific Objective 2:

Expected Outcome 2.1: The narrative documents are elaborated and completed according to the templates provided (narrative description, logical framework, annexes) and following a gender-sensitive approach
Expected Outcome 2.2: The budget is established according to the template(s) provided
Expected Outcome 2.3: The project is amended following review by L’Initiative’s steering committee (if necessary)

Description du projet ou contexte

Launched at the end of 2011, the Initiative is a French facility that complements the Global Fund to Fight AIDS, Tuberculosis and Malaria. It provides technical assistance and support for catalytic projects in some 40 Global Fund recipient countries to improve the effectiveness of Global Fund grants and enhance the health impact of funded programs. In this way, it helps to ensure the effectiveness of the response to pandemics.
Recent evolutions of the Initiative amplify its catalytic effect by strengthening the capacities of health actors, including civil society and research, improving institutional, political and social frameworks, and supporting innovative approaches to pandemics and health systems strengthening.

It is implemented by Expertise France, the international technical cooperation agency, with funding from the French Ministry of Europe and Foreign Affairs, which oversees the program.

As part of its 2023-2025 strategy, the Initiative is focusing on Human Resources for Health (HRH), and in particular on training (both pre-service and in-service), in recognition of the relationship between the the three pandemics and the skills of the health workforce. This new priority is being implemented through the Initiative’s usual channels (Expertise Channel and Projects Channel), but also through its new mechanism, L’Accélérateur – which allows for more substantial investments and more flexible modalities, particularly in terms of project development support. Rwanda has been identified as one of the key countries eligible for funding on HRH from L’Accélérateur in 2024-2025.

Like many other countries eligible to L’Initiative, Rwanda’s HRH indicators are well below international recommendations. The number of doctors per 10,000 inhabitants (1.16) is almost 10 times below the threshold deemed critical by the WHO (15 doctors per 10,000 inhabitants). This figure is not offset by the large number of nurses and/or midwives, who are also well below the recommended thresholds.
Nearly 83% of the Rwandan population lives in rural areas. Only 24% of the country’s doctors are specialists, and they work mainly in the Kigali region. The remainder (76%) are general practitioners, working in rural areas, mainly in district hospitals.

Among others, the Human resources management and training systems are confronted with the following challenges:
– Public primary health care facilities and district hospitals face major difficulties in terms of staff retention;
– Initial training programs are struggling to fill up and produce enough trained staff;
– There are few facilities for continuing training – particularly in rural areas.

HRH is a top priority for the Ministry of Health, which has just published its accelerated reform strategy “Towards Quadrupling the current workforce in 4 years (4×4): Uplifting Quantity and Quality of care” – in addition to its HRH Professional Development Strategy 2020-20230 and other strategies at national level (Health Sector Strategy 2018-2024, National Transformation Strategy). As part of this reform, training is the main priority, both pre-service and in-service, in Rwanda and abroad.

The “4×4” reform proposes 4 areas of work:

  1. Improving the quantity and quality of healthcare professionals in the highest priority training programs (medical specialization, midwifery, anesthetic nursing, radiology, dentistry, ophthalmology and biomedical/laboratory analysis). The medical and nursing specialties most in demand are: oncology, cardiology, nephrology and neonatology;
  2. Increase national faculty recruitment and recruit visiting professors to meet immediate teaching needs;
  3. Improve training capacity and teaching sites (including internship sites, infrastructure and student accommodation);
  4. Coordinate the implementation of the reform (Ministry of Health, Health Workforce Development)

Following L’Initiative’s steering committee validation of the eligibility of Rwanda on HRH in March 2024, a project is expected to be developed and submitted for funding before the end of 2024 – with the support of L’Accélérateur’s team.

Profil souhaité

The skills and experience required are as follows:

  1. Training/Education :
  • University degree (graduate level) in one of the following fields:
    • Medicine
    • Public Health
    • Health Services Management
  • Specialization/experience in the field(s) of management/human resources, Health Systems Strengthening (HSS), pre-service/in-service health training programs (medical, paramedical)
  1. Required skills :
  • Confirmed skills in:
    • Communications
    • Teamwork and interpersonal skills
    • Knowledge transfer
    • Supervision
    • Analysis and problem solving
    • Decision-making and initiative
  • Excellent command of English (written/spoken)
  • Good knowledge of French desirable
  • Knowledge pertaining to gender is desirable
  1. General professional experience
  • At least 10 years professional experience in Human Resources for Health field (management, training)
  • 5 to 10 years professional experience in project development and grant writing
  • At least 5 years professional experience in the medical sector in Rwanda including an experience on HIV, Tuberculosis or Malaria
  • Experience of gender mainstreaming interventions in previous assignments and/or projects
  1. Specific professional experience
  • At least 3 years similar experience
  • Professional experience in the country is desirable.
